Half Life - Half Story

IA little knowledge is dangerous, an old adage that still holds true, especially when talking in the murky world of length of NSAID activity.

The idea that NSAIDs can be simply compared on half life ignores other factors that may prolong the activity of a drug in the body.

The half simply is a part of the measure of the decay curve of the drug over time but does not take into account other factors such as continued absorption or even re-absorption.

There is only one NSAID that can demonstrate therapeutic blood levels for as long as 48 hours and that is not because of simple half life.

The two major factors involved are entero-hepatic circulation, whereby the drug is excreted intact in the bile and reabsorbed via the intestine, in an ever decreasing circle, and, more importantly, the “flip flop” phenomenon of absorption.

This latter, despite the simple name, is a highly complex pharmacological process that applies to certain drugs.

It relies on complicated formulae dealing with the rate of absorption against the rate of elimination and so depends on I/M administration meaning that Tolfedine, and only Tolfedine, can give up to 48 hours therapeutic levels in the cow.
